The quote tool

The quote form requests your gender, date of birth, self-reported health status, tobacco use, the coverage amount you want and how many years you need it. These six inputs are sufficient to generate rates.

Applying

If you choose to apply, the answers you gave are used to prefill a protected form run by the carriers' application platform, and you continue under that platform's own privacy terms. Applications are handled by Susman Insurance Agency, a licensed California insurance agency (License 0B75129).

The contact form

The contact form collects your name, email address, an optional phone number, your communication preference and a message. This information is used solely to respond to your inquiry.
